Care Co-ordinators
Care Co-ordinators are one of the new Additional Roles the NHS is implementing - across Rushcliffe they do a lot of behind the scenes work to keep our health service turning.
A lot of work is done behind the scenes to keep our health services running, and Care Co-ordinators help support other healthcare professionals.
They work with the GP practices and across the whole PCN to keep services running and help patients keep on top of their conditions.
Some of their roles include:

Patient searches and data gathering

Organise patient appointments

Support practices to meet specific requirements from NHS England and the ICB

Running the COVID-19 vaccination programme

Supporting the out of hours clinics

Helping new service developments

Writing and running reports
